Most likely real! Nearly all the old 3D films are real.

It's partially in 3-D. Parts that are are real. Device is that an antique mask makes its wearer have compelling hallucinations- those are in 3-D. The 3-D parts are pretty art-school neat. This has been shown on TV as an anaglyph (it's in black and white so that's easy). The film implores you to Put The Mask On Now! when the character does (and the movie switches to 3-D). I have an off-air tape of it somewhere.

It was a Canadian film. Their first horror film. and it has four 3d sequences. When the character in the movie puts on the mask so does the viewer. it is similar to 'flesh and blood show' which has the last fifteen minutes in 3d only. Similar in that regard , not in story. The flesh and blood show" is on blu ray now. with it's 3d in real 3d , the mask's 3d sequences are great though and the movie is a cult classic. It needs a 3d blu ray release. other new classic 3d releases are 'inferno' (1953), man in the dark !1953), house of wax , creature from the black lagoon, dragonfly squadron(1954), and the bubble , all are classic real 3d films shot in 3d and out on 3d blu ray. kiss me kate , will be out in 2015.
